Job summary

Responsibilities

• Serve as the primary liaison between families, guardians, community partners, and the treatment team, ensuring clear communication and coordination throughout treatment.
• Coordinate referrals, assessments, admissions, treatment planning, case conferences, and discharge planning.
• Collaborate with families, providers, and community agencies to support treatment goals and address barriers to progress.
• Provide families with education, resources, and support to strengthen engagement and improve outcomes for children.
• Maintain accurate, timely documentation and ensure compliance with agency, Medicaid, and regulatory requirements.
• Monitor treatment progress, facilitate ongoing review of care plans, and support continuity of services throughout the child's treatment journey.
