Job summary

Qualifications

• Proficiency with Microsoft Office applications and Windows 10 programs.

Responsibilities

• Provide direct case management services to persons served.
• Assess needs and develop goals with persons served/family/support system.
• Maintain accurate and timely records of persons served and coordinate services with teams.
