Job summary

You’re With Us Inc in Worcester, MA is seeking a Community Based Case Manager to support clients in achieving independence, independence in living skills, and active community participation through individualized programs.

The role focuses on case management, skills training, transportation support, and collaboration with guardians and providers to ensure outcomes align with client goals and program standards.

Qualifications

• High school diploma or GED (required).
• Valid driver’s license and acceptable driving record (required).
• Current First Aid, CPR and Crisis Intervention certificates or ability to obtain within 30 days (required).
• Pass criminal background check (CORI) and National Fingerprint Background Check (required).

Responsibilities

• Provide case management and skills training to clients in areas such as career exploration, employment skills, college campus inclusion, communication, decision‑making, social skills, transportation goals, community safety and daily living activities.
• Develop programming and curriculum tailored to each client to reach their goals in a safe, supported environment.
• Review case records and programming to ensure compliance with You’re With Us! standards.
• Provide transportation to participants using the candidate's own vehicle when needed.
• Implement and monitor curriculum and treatment plans, document activities, and report concerns promptly.
• Develop and run group activities in the community.
• Advocate for You’re With Us! clients and their families.
• Maintain positive relationships with guardians, family members and other service providers.
• Perform other related duties as assigned by senior staff.
