Job summary

You’re With Us! is seeking a Community Based Case Manager to support programming for clients, helping them achieve personal goals with independent living skills and community integration.

The role focuses on inclusive activities on college campuses and sustaining opportunities with ongoing coaching and advocacy. Responsibilities include case management, curriculum development, and coordinating transportation and community activities while ensuring compliance with organizational standards.

Qualifications

• Must have a High School Diploma or GED.
• Valid driver’s license with acceptable driving record.
• Current First Aid, CPR and Crisis Intervention certificates or able to obtain within 30 days of hire.
• Pass criminal background check (CORI) and National Fingerprint Background Check.

Responsibilities

• Provide case management and skills training support across career exploration, employment skills, college campus inclusion, communication, decision-making, social skills, transportation, safety and daily living activities.
• Develop programming and curriculum tailored to individual client goals in a safe, supportive environment.
• Review case records regularly to ensure compliance with You’re With Us! standards.
• Provide transportation to participants in the candidate’s own vehicle as needed.
• Document and report significant concerns in a timely manner to You’re With Us!
• Develop and run community group activities and advocate for clients and families.
• Maintain positive relationships with guardians, family members and other service providers.
