Description

Zepf Center has been serving the Lucas County community for 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a Sanctuary trauma-informed agency and environment for both patients and staff.

We are currently seeking a Full-Time Community Based Case Manager. The purpose of this position is to provide a wide range of community support and coordination of services to people experiencing serious mental illness to ensure their needs are addressed. The ACT Case Manager is responsible for case management processes including problem identification, assessment, care planning, interventions, evaluation, and revision of plans, in accordance with ACT fidelity requirements. The Case Manager works in the community, at clients' homes, and agency locations. Works hours/shifts as posted.

Essential Duties And Responsibilities

  • Assess client strengths and needs, conduct care coordination, and develop individualized treatment plans.
  • Participate in daily ACT team meetings for planning and task implementation.
  • Monitor and evaluate client progress; review and modify treatment plans as needed.
  • Maintain contact with clients to ensure stability.
  • Provide outreach, crisis intervention, and community support.
  • Refer clients to services and assist with entitlements; advocate for clients; communicate with families as needed.
  • Document services and billing accurately and timely.
  • Provide on-call coverage during evenings and weekends as required.
  • Perform other duties as assigned.

Requirements

  • Bachelor's degree in a related field.
  • At least 2 years of experience working with individuals with severe and persistent mental illness.
  • QHMS certification required; CDCA preferred.
  • Valid Driver's License and ability to drive safely in all weather conditions.
  • Knowledge of community resources and mental health services.
  • CPR, CPI, Sanctuary, and annual booster training required.
  • Excellent communication skills.
