Turning Point Community Programs in Sacramento is seeking a Mental Health Rehabilitation Specialist to provide psychosocial, rehabilitation and recovery-oriented services to adults in the community.

You will advocate for clients, coordinate with families and service providers, maintain records, and assist with Medi-Cal eligibility, housing, and employment supports. This role emphasizes compassionate outreach and effective collaboration with a multidisciplinary team.

Qualifications

• Bachelor's degree in Social Work or related field is required or preferred.
• 0-2 years varied mental health experience is required or preferred.
• California driver's license with current vehicle insurance/registration is required.
• Reliable transportation and ability to pass vehicle safety inspection if the vehicle is older than five years.

Responsabilités

• Provide leadership and direction for clients as assigned when needed.
• Advocate for clients to access services and empower them to meet goals.
• Assist clients and families in obtaining mental health treatment, housing, food, and clothing.
• Maintain client records and complete required documentation and data entry.
• Meet with clients at Turning Point sites and other approved locations.
• Coordinate with families and care providers with client consent.
• Coach clients in social skills, money management, personal hygiene, and job readiness.
• Assist in Medi-Cal eligibility and continuation of benefits where appropriate.
• Develop financial management plans for clients receiving Representative Payee Services.
• Collaborate with the team to deliver holistic case management services.
