Job summary

SCMHCC in Augusta, KS seeks an Adult Case Manager to serve adults with serious mental illness in a Community Support Program. You will apply the Strengths Model and IDDT to guide recovery, deliver community-based services, and collaborate with a multidisciplinary team to coordinate care.

You will develop recovery plans, assess strengths, and advocate for clients, while ensuring timely notes and compliance with program standards.

Qualifications

• BA/BS in human services, psychology, social work, or related field or equivalent.
• One year of education substitution allowed for one year of education.
• Experience with serious emotional disturbances preferred.

Responsibilities

• Develop continuous, supportive relationships with clients in the Community Support Program.
• Conduct Strengths Assessments and co-develop Personal Recovery Plans.
• Deliver direct client services in community and non-office settings.
• Review treatment plans quarterly to monitor progress and quality of care.
• Apply IDDT model interventions including counseling and solution-focused techniques.
• Help clients understand symptoms and co-occurring disorders and minimize impact.
• Facilitate client autonomy and community integration; build natural support networks.
• Advocate for clients and coordinate referrals with multidisciplinary teams.
• Document progress notes promptly for billed codes.
