Job summary

EPIC Health Partners, LLC is seeking an ACT Clinician to join our Assertive Community Treatment team in Danville, VA. The clinician will provide community-based mental health services to adults with serious mental illness, working with a multidisciplinary team to deliver assessment, treatment, rehabilitation, care coordination, and crisis support to promote recovery.

Qualifications

• Master’s degree required in counseling, social work, psychology, or related field.
• Licensed or license-eligible in Virginia as LPC, LCSW, LMFT, or Clinical Psychologist.
• Minimum of 1 year of clinical experience including assessments, treatment planning, therapy services, and crisis/behavioral interventions.
• Valid driver’s license and safe driving record required; Virginia Pre-Screener Certification within six months of hire.

Responsibilities

• Conduct assessments and develop/implement individualized treatment plans.
• Provide counseling, rehabilitation, crisis intervention, and support services.
• Deliver services in community settings, including homes and other environments.
• Coordinate care with ACT team, medical providers, family, and community resources.
• Help individuals develop skills for recovery, independence, and community integration.
• Maintain clinical documentation per regulatory and agency requirements.
• Assist with weekly medication bag/box fills and medications runs.
• Attend team meetings, trainings, and treatment planning sessions.
• Complete progress notes, treatment plans, quarterly reviews, and service transitions.
