Spring Dell Center, Inc. seeks a committed Community and Employment Professional to assist individuals with community activities and workplace support in Charles County, MD. This role involves creating personalized plans, documenting progress, and helping participants discover meaningful employment opportunities.

The ideal candidate will be patient, organized, motivated, and a self-starter, capable of coordinating supports and scheduling activities that align with each person’s goals.

Qualifications

High school diploma or GED required.
Ability to lift 50 lbs or more.
Valid driver's license with clean record.
Basic computer skills for daily tasks.

Responsibilities

Support people with exploring activities in their local community and at work.
Develop personalized plans and list meaningful opportunities with staff support.
Document progress toward outcomes and goals.
Plan activities to expand community relationships and social skills.
Coordinate supports in the community 5 days per week.
Assist with discovery to identify interests related to activities and employment.

Community and Employment Professional

Hourly Salary: $19.64/hr

Status: Full-Time

Shift: Monday - Friday (hours varies from 7:30 am - 3:30 pm)

Location: Charles County, MD

Job Description:
To support people with exploring activities in their local community and/or support while at their jobs. The perfect applicant will enjoy supportingpeople in trying new opportunities in the community, developing meaningful relationships with community members, maintaining employment anddiscovering other meaningful activities or employment of the person’s choice, while achieving their goals. Preference will be given to the person who can demonstrate the abilities to be patient,organized, motivated, and a self-starter. The applicant will have the ability to create a personalized schedule for the people supported and listing meaningful opportunities that they choose with staff support. Desired character traits include role model, being compassionate,pro-active, observant, caring and patient. Duties Include but not limited to:

Implementing the interest and likes of the person(s) supported.
Monitoring and evaluating their interests and needs.
Assisting with the development of the “Personalized Plan(s)” and achieving established outcomes/goals for the person(s).
Documenting the progress of outcomes/goals and/or other supports as listed the person’s Individualized Plan(s).
Planning activities around areas that may peak their interest in community engagement.
Planning activities that will provide the person(s) with opportunities to expand theircommunity relationships and social skills.
Actively participating in all aspects of the person(s) supports.
Coordinating supports in the community 5 days per week.
Support provided while at work.
Identifying possible employment in the community of the person’s interest.
Assisting with Discovery process to identify a person’s passions and interests related to meaningful activities and employment.
