Public Health Solutions administers WholeYouNYC, a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WholeYouNYC brings together over 200 organizations offering programs across all five boroughs that include food, housing, employment, health insurance, and sexual health services. These services help New Yorkers live healthier lives and reduce health disparities. To date, the network has impacted thousands of lives through partnerships and referrals, generating millions in estimated healthcare savings. New York State (NYS) invested $500M statewide to support Social Care Network (SCN) lead entities integrating social care into Medicaid across the state. PHS’ WholeYouNYC was awarded the Social Care Network infrastructure grant for Brooklyn, Queens, and Manhattan. This is a grant-funded position ending March 31, 2027.
The Development and Communications Department at Public Health Solutions plays a pivotal role in advancing our mission to improve community health through innovative programs and strategic partnerships. This department generates resources, builds relationships, and promotes our initiatives to maximize impact. Through fundraising, marketing, and effective communications, the department ensures the sustainability and visibility of our programs. Its key functions include fundraising and resource development, grant management, donor engagement, campaign management, marketing and communications, strategic marketing, content development, brand management, public relations, community and stakeholder engagement, and partnership development. The goals are to increase funding, expand reach, strengthen relationships, and enhance communication. The department comprises experienced professionals in fundraising, marketing, communications, and public relations who work collaboratively to drive the success of Public Health Solutions’ initiatives.
