Job summary

VNS Health is seeking a licensed Physical Therapist to provide 1:1 home-based rehabilitation across New York. You will assess patients, design individual treatment plans, and deliver therapy in the comfort of patients’ homes.

Qualifications

• Current New York State Physical Therapist License and Registration required.
• Minimum of one year PT experience preferred.
• Valid driver's license may be required depending on territory.

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Deliver personalized home-based physical therapy services.
• Monitor patient progress and adjust plans of care as appropriate.

What You Will Do

Qualifications

• Minimum of one (1) year of Physical Therapy experience preferred, or successful completion of the VNS Health Rehab Intern program.
• Current New York State Physical Therapist License and Registration required.
• Valid driver's license may be required based on assigned territory.

Pay Range per Visit:

• Bronx & Westchester: Weekday visits - $105 | Weekend visits - $110
