VNS Health is seeking a committed Per Diem Physical Therapist for home care in the Bronx. You will deliver one-on-one, home-based therapy to patients recovering from surgery, illness, or injury, designing personalized treatment plans and evaluating progress with an interdisciplinary team.

Qualifications

• Minimum of one year of Physical Therapy experience preferred, or successful completion of the VNS Health Rehab Intern program.
• Current New York State Physical Therapist License and Registration required.
• Valid driver’s license may be required based on assigned territory.

• Practice independently in the community as part of an interdisciplinary care team.
• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Deliver personalized home-based physical therapy services.
• Monitor patient progress and adjust plans of care as appropriate.

• Collaborate with physicians and interdisciplinary team members.
