Job summary

Med Center Health in Scottsville, KY seeks an LPN to provide direct patient care under the supervision of a Registered Nurse. Completion of an LPN program and Kentucky licensure are required, with prior healthcare experience preferred.

Qualifications

• Completion of Licensed Practical Nurse program required.
• Currently licensed as an LPN or LVN in the state of Kentucky required.
• Previous healthcare experience preferred.

Responsibilities

• Partners with Registered Nurse to advance the patient’s plan of care.
• Provides safe, high quality, individualized care to patients.
• Assigns tasks to unlicensed assistive personnel in accordance with policy parameters.
• Recognizes when there is need for assessment or further intervention and escalates to the RN.
• Establishes a caregiver-patient relationship built on trust, respect, compassion and communication.
• Responds to the clinical and service needs of patients and families proactively.
