Job summary

Group CBS is seeking a Licensed Practical Nurse to join our clinical team in Kentucky. You will assist in nursing history, develop and update care plans, and collaborate with the RN and other disciplines to ensure safe patient care.

You will administer medications, monitor patient responses, document accurately, and participate in discharge planning. The role emphasizes patient education, safety, and adherence to licensed scope of practice.

Qualifications

• Graduate from an approved school of Nursing; LPN licensure required.
• Licensed Practical Nurses must be licensed by the state of Kentucky.
• Experience with standard nursing procedures and patient care roles preferred.

Responsibilities

• Assist in completion of nursing history and care plans.
• Administer medications following the 5 rights and physician orders.
• Document interventions accurately and timely in patient records.
• Perform pre-procedure assessments and vital signs; monitor patient status.
• Support discharge planning and coordinate with RN and team.
• Provide health teaching to patients and caregivers as needed.
