Job Description

Performs the essential duties of a Coder/Abstractor, including coding hospital medical records for patient care, reimbursement, and research according to coding guidelines. Codes diagnoses, operations, and procedures using ICD-10-CM/PCS and CPT systems. Handles inpatient, outpatient, observation, same-day surgery, ER, and urgent care records for billing and data retrieval. Abstracts clinical and demographic data following hospital policies. Solves practical problems and interprets instructions, demonstrating clerical knowledge and judgment.

Shift and Schedule

Monday - Friday, 8:00 AM - 5:00 PM. This position is remote.

Essential Functions

• Adheres to ICD-10-CM/PCS and CPT guidelines, demonstrating understanding of coding conventions.
• Follows UHDDS definitions for diagnoses and procedures.
• Participates in coding meetings and educational conferences to maintain accuracy.
• Abstracts relevant clinical and demographic data from medical records.
• Productivity standards: Inpatient charts=4/hr; Observation/Same Day Surgery=6/hr; Outpatient/Lab/Xray=20/hr; ER=20/hr.

Education and Experience

• High school diploma or equivalent, capable of solving practical problems.
• Preferred: 6 months to 1 year experience in Health Information Management or related setting.
• Preferred: CCS or RHIT certification.
• Keyboarding and clerical experience.
