Job summary

Ascendion is seeking an experienced Medical Coder to join our remote team. You will review clinical documentation, assign ICD-10-CM diagnosis codes and CPT procedure codes, and ensure coding accuracy and compliance for outpatient encounters.

Ideal candidates have 2–4 years of medical coding experience, CPC/CCS certification, and strong knowledge of medical necessity and E/M coding. This role offers remote flexibility and comprehensive benefits.

Qualifications

About the Role:

We are seeking experienced Medical Coders with strong knowledge of ICD-10-CM diagnosis coding and CPT procedure coding. The ideal candidate will review clinical documentation, validate medical codes, and ensure coding accuracy and compliance with applicable coding guidelines.

Job Title: Medical Coder

Key Responsibilities:

• Review clinical documentation and assign accurate ICD-10-CM diagnosis codes and CPT procedure codes.
• Perform coding for primary care and outpatient physician encounters.
• Review medical records to ensure coding accuracy, completeness, and compliance.
• Validate codes against clinical documentation and applicable coding guidelines.
• Evaluate Evaluation and Management (E/M) coding and medical necessity.
• Identify coding discrepancies, documentation gaps, and potential compliance issues.
• Support coding audits, quality reviews, and coding validation activities.
• Maintain accurate and consistent coding practices in accordance with industry standards.

Minimum Qualifications:

• 2–4 years of medical coding experience.
• Hands‑on experience with ICD-10-CM and CPT coding.
• Experience with outpatient, primary care, or physician professional‑fee coding.
• Active CPC, CCS, or equivalent medical coding certification.
• Knowledge of E/M coding, medical necessity, and clinical documentation review.
• Strong attention to detail and ability to work independently in a remote environment.

Desired Qualifications:

• Experience in medical coding audits, coding compliance, or quality assurance.
• Payer‑side experience, including claims review, payment integrity, or claims auditing.
• Experience reviewing and validating clinical documentation and coding accuracy.
• Familiarity with AI‑generated clinical summaries, coding validation, or healthcare data annotation.

Location: Remote

Salary Range:

The salaryfor this position is between$45,000 – $50,000 annually. Factors which may affect pay within this range may include geography/market, skills, education, experience, and other qualifications of the successful candidate.
