Job summary

Positive Pathways Family Services, LLC is seeking compassionate Community Engagement Coaches to empower adults with intellectual and developmental disabilities to live meaningful, independent lives within their communities.

As a Community Engagement Coach, you will provide one-on-one support that promotes independence, social inclusion, and daily living skills while ensuring health, safety, and wellbeing. The role emphasizes dignity, choice, and community participation.

Qualifications

  • High school diploma or GED required.
  • Experience with individuals with IDD or related human services preferred.
  • Current CPR/First Aid certification (or willingness to obtain).

Responsibilities

  • Provide one-on-one support in community settings per ISP.
  • Encourage participation in community activities and opportunities.
  • Develop communication, decision-making, self-advocacy, and problem-solving skills.
  • Support independent living skills: budgeting, shopping, meal planning, transportation, and navigation.
  • Observe and report changes in health, behavior, or safety.
  • Complete accurate service notes within required timeframes.

Positive Pathways Family Services, LLC is seeking compassionate, dependable, and motivated Community Engagement Coaches to empower adults with intellectual and developmental disabilities to live meaningful, independent, and connected lives within their communities.

As a Community Engagement Coach, you will provide one-on-one support that promotes independence, social inclusion, community participation, relationship building, and the development of daily living skills while ensuring the health, safety, and well-being of the individuals we serve.

Position Summary

The Community Engagement Coach provides individualized support in community settings based on each person's Person-Centered Individual Support Plan (ISP). Coaches assist individuals in building natural supports, developing life skills, participating in meaningful community activities, and achieving personal goals while promoting dignity, choice, and independence.

Essential Responsibilities
  • Provide one-on-one support in community settings in accordance with the individual's ISP and service authorization.
  • Encourage meaningful participation in community activities, recreation, volunteer opportunities, educational experiences, and employment exploration.
  • Assist individuals in developing communication, decision-making, self-advocacy, and problem-solving skills.
  • Support the development of independent living skills, including budgeting, shopping, meal planning, transportation, and community navigation.
  • Promote healthy relationships and community inclusion while encouraging independence rather than dependence.
  • Assist individuals with identifying and achieving personal goals.
  • Implement behavior support strategies and follow individualized risk management plans when applicable.
  • Observe and promptly report changes in health, behavior, or safety concerns.
  • Complete accurate service documentation and daily notes within required timeframes.
  • Maintain confidentiality and comply with HIPAA, DBHDS regulations, Medicaid requirements, and Positive Pathways Family Services policies.
  • Participate in required trainings, staff meetings, and ongoing professional development.
  • Transport individuals safely to community activities when required.
Qualifications
  • High school diploma or GED required.
  • Experience working with individuals with intellectual or developmental disabilities, behavioral health, education, healthcare, or human services preferred.
  • Excellent interpersonal and communication skills.
  • Ability to work independently and exercise sound judgment.
  • Reliable transportation, valid driver's license, and acceptable driving record.
  • Ability to successfully complete background checks and all required pre-employment screenings.
  • Current CPR/First Aid certification (or willingness to obtain).
  • Ability to lift up to 25 pounds and assist individuals with community mobility when necessary.
Preferred Qualifications
  • Experience supporting individuals receiving Virginia Developmental Disabilities Waiver services.
  • Knowledge of person-centered practices and positive behavioral supports.
  • Medication Aide, DSP, CNA, QMHP, or related human services experience is a plus.
