Job summary

Cedar Lake Wellness & Rehab is seeking a dedicated CNA-oriented transportation coordinator. You will organize and manage resident transport to physician appointments, maintain safety protocols, and support clinical teams with transfers and documentation.

This role emphasizes timely, respectful service to residents, with strong adherence to safety policies and cooperation with nursing staff to ensure seamless daily operations.

Qualifications

• Valid driver’s license in the state where you are applying.
• Completion of State Approved Nurse Aide Training and Competency Evaluation Program preferred but not required.
• Current BLS/CPR certification (required by many SNFs).

Responsibilities

• Coordinate transportation of residents to appointments per priority of needs.
• Transport residents to inpatient and outpatient programs and to appropriate facilities.
• Develop driving schedules based on resident appointments and needs; assist with hospital discharges as needed.
• Provide status updates and report any changes in resident condition to supervisors.
• Transport medical records for signatures as required and maintain vehicle safety and cleanliness.
• Assist clinical staff with safe lifting, transfers, and transports per physician orders.
