Job summary

Atrium Centers is seeking a Transport Driver/CNA in Wisconsin. You’ll schedule and provide transportation for residents, assist during transport and appointments, and drive a center van up to two patients at a time.

You’ll split duties 50/50 between floor CNA responsibilities and transport driving, and follow Care Plans to deliver resident care. Requirements include a completed CNA course with active state license, 25+ age, clean driving record, high school diploma or GED, and preferred Medical

• Clean driving record and valid driver's license.

Responsibilities

• Schedule and/or provide transportation for residents to and from appointments.
• Assist residents safely during transport and appointments.
• Drive center van and transport up to two patients at a time.
• Split duties 50/50 between CNA responsibilities and transport driving.
• Provide resident care according to approved Care Plan.
• Assist with daily activities, personal care, and resident safety.
• Maintain and respond to resident medical record requests.
• Follow infection control procedures and safety protocols.
• Complete required documentation, including vitals.
• Report any violations of Residents' Bill of Rights immediately.

Additional Offerings

• Full Time: 1st Shift
• $21 - $22 / Hour (Based on Experience)
• MUST be a Certified CNA

What You’ll Do

• Schedule and/or provide transportation for residents to and from appointments
• Assist residents safely during transport and appointments
• Drive center van (similar in size to a minivan)
• Transport a maximum of two patients at a time
• Split duties 50/50 between floor CNA responsibilities and transport driving
• Provide resident care according to an approved Care Plan
• Assist with daily activities, personal care, and resident safety
• Maintain and respond to resident medical record requests
• Follow infection control procedures and safety protocols
• Complete required documentation, including vitals
• Report any violations of the Residents' Bill of Rights immediately

What You Bring

• Completed CNA course and active state license
• Must be 25 years of age or older
• Clean driving record
• High school diploma or GED
• Medical Records experience in a long-term care setting (preferred)
• ART certification (preferred)
• Strong written and verbal communication skills
• Service-oriented mindset with a willingness to learn
• Positive, can-do attitude and excellent attendance
