JOB SUMMARY:

• Participates in New York State approved training program of at least 75 hours duration, which includes both classroom and clinical training:
• While engaged in clinical training, the trainee can only provide care under the supervision of the clinical instructor/supervisor.
• The trainee must be scheduled for the state competency examination within 10 days of successful program completion.
• The trainee may only be employed as a nurse aide trainee for 120 days from the date of hire or the first date of the nurse aide training program, whichever comes first as required per regulation.
• Candidates have three (3) attempts to pass the New York State authorized residential health care facility nurse aide Written and Clinical skills competency examinations within 120 days of employment or the first day of training.
• Assist or complete resident activities of daily living according to the care plan for assigned residents.
• Activities of Daily Living include, but are not limited to the following:
• Bathing (tub, shower, bed)
• Dressing
• Transferring (in and out of bed, chair, wheelchair, lounge chair, tub, scale, etc.)
• Transporting (wheelchair, walker, ambulation)
• Lifting
• Range of Motion exercises
• Bed making
• AM and HS care
• Provision of privacy
• Restorative Nursing measures
• Safely use equipment (lifts, scales, restraints, side rails, splints, positioning devices, arm protectors, specialized mattresses, adaptive equipment)
• Obtain and record vital signs(temperature, pulse, respiration) and weight.
• Maintenance of dignity, individuality, confidentiality and resident’s rights
• Explain to resident what care you are going to provide prior to providing it.
• Caring for residents with communicable diseases who require isolation or special precautions. Wearing appropriate personal protective equipment and following infection control procedures.
• Maintaining resident’s room, bath and personal belongings in a clean, orderly fashion.
• Assists with admissions and discharges as directed by the nurse.
• Reports to the team leader at the beginning of the shift, listens to report of the resident’s condition, receives assignment, asks questions and reviews the resident’s care plan.
• Reports off to team leader when leaving the unit.
• Adheres to assigned break and meal times.
• Attends in-services as directed
• Reports perceived problems to the supervisor
• Is responsible for any duties, within the scope of practice of a CNA, as requested by the team leader, nursing supervisor, ADON, DON.
• Follows the facilities policies and procedures
• Follows Universal Precautions and Infection Control practices
• Maintains Corporate Compliance standards
• Contributes to facility Quality Assurance Performance Improvement initiatives
