Job summary

Hospice Austin is seeking an attentive Certified Nursing Assistant to provide direct patient care under RN supervision in a hospice setting. You will follow the established Plan of Care, assist with daily activities, monitor patient condition, and support family members in care routines.

Responsibilities include essential hygiene, mobility assistance, vital signs, documentation, and collaboration with the clinical team.

Qualifications

• Current, active Texas nurse aide certification (CNA).
• Currently listed in good standing on the Texas nurse aide/misconduct registry, with no finding against the aide relating to client abuse or neglect or misappropriation of client property, as verified through the Texas Department of Human Services (DHS).
• Valid Texas driver’s license, auto liability insurance and safe driving record.
• CPR certification.

Responsibilities

• Provides personal care per plan of care, including bathing, grooming, oral hygiene, and skin checks.
• Assists with transfers, turning, and safety during mobility.
• Takes vital signs and monitors weights as directed.
• Provides care for medications that are ordinarily self-administered and documents care.
• Communicates changes to RN Case Manager and documents in patient records.
• Obtains supplies, teaches use of medical supplies, and ensures equipment safety.
• Provides nutritional support and light meal preparation as indicated.
