The primary purpose is to provide each assigned patient with routine daily nursing care and services in accordance with the patient’s assessment, plan of care, and care plan, as directed by supervisors.
Minimum Qualifications
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
- Current WV Certified Nursing Assistant Certification, or applicable state licensure where services will be performed, must be obtained within 3 months of hire.
- Successfully pass WV DHHR Protected Services Record Check and WV CARES fingerprint-based criminal background check (if applicable).
- Obtain certification in Basic Life Support within 30 days of hire.
- High school education or GED required.
Experience
PREFERRED QUALIFICATIONS:
- One year nursing assistant experience in long-term care preferred.
Core Duties and Responsibilities
The following are general responsibilities; other duties may be assigned:
- Record all entries accurately on flow sheets, notes, charts, etc., per policies and procedures.
- Measure and record vital signs such as temperature, pulse, and respirations.
- Provide care based on physical, psychosocial, communication, safety, and educational needs of adult geriatric patients.
- Use equipment like vital signs instruments, scales, oxygen tanks, drainage systems, SCDs, CPM machine, and bladder scanner according to manual.
- Assist patients with hygiene, nail care (excluding diabetics), and other daily activities.
- Turn and position bedfast patients every 2 hours and as needed.
- Assist with bowel and bladder functions, including retraining programs.
- Respond promptly to call bells.
- Provide care for dying patients and postmortem care as per policy.
- Perform catheter care and assist with activities of daily living.
- Serve and assist with feeding, record intake, and help with food arrangements for visually impaired patients.
- Assist with patient mobility, including lifting, turning, and transporting, using proper body mechanics.
- Report any abuse, property misappropriation, accidents, or incidents observed during the shift.
- Assist with specialized feeding techniques and gait training as instructed.
- Apply orthopedic devices per physical therapist instructions.
- Use safety equipment when moving patients requiring assistance.
Physical Requirements
- Frequent walking, standing, stooping, reaching, lifting, and other body movements.
- Must be able to lift and move patients of all weights with assistance.
- Normal hearing and visual acuity required.
- Ability to exert force exceeding 100 pounds occasionally, 50 pounds frequently, and 20 pounds constantly.
Work Environment
- Exposure to body fluids, tissues, contagious diseases, sharps, gases, cleaning agents, and odors.
- High-stress environment with interruptions.
- Work includes weekends, holidays, and various shifts.
Skills and Abilities
- Ability to understand and follow oral and written instructions.
- Effective communication skills.
- Math skills for medication calculations.
Additional Job Details
Full-Time, 3-12 hour shifts, Days or Nights, 7 am - 7 pm or 7 pm - 7 am.
