Primary Purpose

The primary purpose is to provide each assigned patient with routine daily nursing care and services in accordance with the patient's assessment, plan of care, and care plan, as directed by supervisors.

Minimum Qualifications
Education, Certification, and/or Licensure:
  1. Current WV Certified Nursing Assistant Certification, or applicable state licensure where services will be performed, must be obtained within 3 months of hire.
  2. Successfully pass WV DHHR Protected Services Record Check and WV CARES fingerprint-based criminal background check (where applicable).
  3. Obtain certification in Basic Life Support within 30 days of hire date.
  4. High school education or GED required.
Preferred Qualifications
Experience:
  1. One year nursing assistant experience in long-term care preferred.
Core Duties and Responsibilities

The statements here describe the general nature of work and are not exhaustive. Other duties may be assigned.

  1. Record all entries accurately on flow sheets, notes, charts, etc., per policy/procedure.
  2. Measure and record temperatures, pulse, and respirations as per policy/procedure.
  3. Provide care based on physical, psychosocial, communication, safety, and educational needs of adult geriatric patients, following established standards.
  4. Use equipment such as vital sign instruments, scales, oxygen tanks, drainage systems, SCDs, CPM machine, and bladder scanner according to manual.
  5. Assist patients with daily hygiene and personal care.
  6. Assist with nail care, excluding diabetic patients per policy.
  7. Turn and position bedfast patients every 2 hours and as needed, assist with bowel and bladder functions, and participate in bladder and bowel retraining programs.
  8. Respond promptly to call bells per policy/procedure.
  9. Assist with care of dying patients and provide postmortem care per policy.
  10. Provide catheter care as necessary.
  11. Assist with activities of daily living (ADL) with or without adaptive equipment.
  12. Serve and assist with feeding, record food and fluid intake, and help patients with food identification if needed.
  13. Serve snacks between meals and at bedtime.
  14. Assist with patient mobility, including lifting, turning, moving, and transporting, using proper body mechanics.
  15. Report all patient abuse or property misappropriation as per policy.
  16. Report accidents and incidents observed during the shift.
  17. Assist with specialized feeding techniques and ambulation as instructed by therapists.
  18. Apply orthopedic devices per physical therapist instructions.
  19. Use safety equipment when lifting or moving patients requiring assistance.
Physical Requirements
  1. Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling.
  2. Ability to lift and move patients of all weights with assistance as needed.
  3. Normal hearing and visual acuity required.
  4. Perform heavy work, exerting force up to 100 pounds occasionally, 50 pounds frequently, and 20 pounds constantly.
Working Environment
  1. Exposure to body fluids, tissues, contagious diseases, sharps, and gases.
  2. Exposure to cleaning agents, disinfectants, toxic gases, fumes, odors, and electrical currents.
  3. Work may involve high stress and interruptions.
  4. Work on weekends, holidays, and various shifts.
Skills and Abilities
  1. Ability to understand and follow instructions.
  2. Effective reading, comprehension, and communication skills.
  3. Math skills for medication calculations.
