As a Hospice CNA, you will play a crucial role in delivering high-quality, personalized end-of-life care to patients in their homes, ensuring their comfort, dignity, and overall well‑being during this sensitive phase of life.

Responsibilities

• Provide hands‑on assistance with activities of daily living (ADLs), including bathing, dressing, grooming, and monitoring vital signs.
• Document any changes in the patient’s condition and report concerns to the registered nurse or hospice team.
• Assist in administering medications as per prescribed treatment plans and report any adverse reactions or side effects.
• Maintain accurate records of medication administration.
• Maintain a clean, safe, and comfortable environment, making necessary adjustments to ensure patient well‑being.
• Implement appropriate pain management techniques as outlined in the patient’s care plan.
• Maintain detailed patient records, documenting care provided, observations, and any changes in the patient’s condition.
• Ensure compliance with all documentation and reporting requirements as per hospice regulations.
• Work collaboratively with the interdisciplinary team, including nurses, social workers, chaplains, and volunteers, to deliver holistic care to patients.
• Participate actively in team meetings and contribute to care planning discussions.

Qualifications

• 1+ years' experience - preferred

• Ability to travel - must have reliable transportation
