Job summary

Douglas County Visiting Nurses is seeking a CNA to provide personal care and related services to patients in accordance with the Plan of Care and agency policies. The role emphasizes safety, comfort, and independence, with charting and adherence to regulatory requirements.

You will support families, assist with activities of daily living, and follow directions from the Case Manager to promote patient well-being in the Lawrence area.

Qualifications

• High school diploma or GED preferred, but not required.
• Certified Nurses Aide in good standing - required for Help at Home program.

Responsibilities

• Assist patients with bathing, dressing, and grooming per the plan of care.
• Demonstrate techniques to help families lift, turn, and reposition patients.
• Support families to maintain normal activities of daily living.
• Encourage patient independence and re-establish daily living activities.
• Observe incidental needs of the patient.
• Exercise initiative and judgment in duties.
• Maintain charting and records per agency and state/federal regulations.
• See referred patients as directed by the Case Manager.

Job description

The CNA is responsible for providing personal care and other services outlined in the Plan of Care to patients/clients in a manner conducive to their safety and comfort. Care is provided in accordance with the policies of the agency and the nursing plan of care for the patient/client and the manager.

• Skillfully assist patients by providing personal services, such as, bathing, dressing, and grooming as outlined on the plan of care necessary to prevent or postpone hospitalization.
• Helps family members care for the patient by demonstrating appropriate ways to lift, turn, and re-position the patient.
• Assist families to maintain their normal and usual activities of life.
• Assist in re-establishing activities of daily living encouraging as much independence as possible.
• Observe for incidental needs of the patient/client.
• Exercise initiative and judgment in performance of duties.
• Maintain charting and records as required by the agency and federal and state regulations, assuring that charting follows the plan of care established by the case manager.
• See referred patients as directed by the Case Manager.

QUALIFICATIONS

• Graduation from high school or completion of GED preferred, but not required.
• Certified Nurses Aide in good standing - required for Help at Home program.

EXPERIENCE

• Basic level knowledge of EMR (electronic medical records) and email functions.

Visiting Nurses requires a criminal background check and motor vehicle (MVR) check for all employees once a conditional offer of employment has been extended by the hiring manager.
