Responsibilities

• Provide high-quality, patient-centered care to clients in their homes, assisting with daily living activities, personal care, and mobility needs.
• Assist with vital signs, medication administration, and wound care under the supervision of a licensed nurse.
• Maintain accurate and detailed records of patient care, progress, and concerns.
• Collaborate with other healthcare professionals to develop and implement personalized care plans.
• Demonstrate a strong commitment to patient safety, confidentiality, and dignity.
