Qualifications

• QMAP certified required; VOANS may provide certification after hire.
• Must be 18 years or older.
• Experience in a Home Health Agency or health care facility strongly preferred.
• CNA or PCA preferred.

Responsibilities

• Provide routine daily nursing care to assigned clients per agency procedures.
• Report any changes in client condition to supervisor.
• Assist with personal care activities, ambulation, transfers and exercises.
• Perform household services essential to health care at home.
• Document cares and observations accurately.
