Qualifications

• Strong communication and teamwork with care team.
• Ability to follow care plans and document needs.

Responsibilities

• Provide personal care such as bathing, grooming, dressing and hygiene.
• Assist with mobility, transfers and safety precautions.
• Medication reminders and monitoring health indicators as directed.
• Prepare meals and assist with light housekeeping as needed.
• Provide companionship and support to maintain independence at home.
