Cantex Continuing Care Network is seeking a Certified Nursing Assistant (CNA) to provide direct patient care in accordance with individualized care plans, applicable policies, and daily nursing support.

The role requires current state CNA certification, ability to read, write and speak English, and physical capacity to lift, move, and transfer patients safely; you will work with nursing staff to promote safety and quality outcomes.

• Current state certification as a Nursing Assistant.
• Ability to read, write, and speak the English language.
• Ability to lift, move, and transfer patients safely.

Responsabilidades

• Provide personal care services to assigned patients in accordance with care plans, safety procedures, and applicable regulations.
• Document patient care activities and observations in accordance with established policies and procedures.
• Promote and support patient independence and quality of life whenever possible.
• Communicate patient status updates and care concerns to the supervising Charge Nurse at the beginning and end of each shift.
• Observe patient conditions, contribute relevant clinical information, and document observations as required.
• Maintain a safe, clean, and hazard-free environment for patients, visitors, and staff.
• Follow established procedures for safe lifting, positioning, transferring, and assisting patients.
• Ensure care-related equipment and mobile devices remain clean, functional, and properly charged, and report malfunctions to the supervisor.
• Participate in required in-service education, certification maintenance activities, and Patient Care Management Systems training.
• Perform incidental housekeeping or maintenance tasks necessary to support a clean and safe care environment.
• Serve as a team leader and role model while supporting abuse prevention, patient safety, and quality care initiatives.
• Perform other duties as assigned.
