Job summary

Peters Township Post Acute is seeking a part-time CNA to provide routine nursing care to residents at our 140-bed facility. You will follow care plans, identify residents by ID, and work with the care team to maintain a warm, safe environment.

You will report changes to the Nurse Supervisor, document accurately, assist with mobility and daily activities, and support hospital-transition and discharge processes. This role includes shift-based hours and access to our benefits package.

Qualifications

• Must possess CNA license in the state.
• CPR certification preferred.
• Minimum 10th grade education.

Responsibilities

• Provide routine daily nursing care per resident care plans.
• Identify residents before treatments and meals using wristband/photo ID.
• Report changes in residents' condition to nurse supervisor promptly.
• Record entries clearly on charts and flow sheets.
• Assist with transfers, mobility, and daily activities as directed.
• Maintain residents' room readiness and comfort.
• Protect resident privacy and report any HIPAA concerns.
• Follow policies on exposure to blood and body fluids.
