Job summary

Lower Cape Fear LifeCare is seeking a Hospice CNA in Bolivia, NC. You will provide direct patient care in homes and facilities, chart in EMR, and coordinate visits with RNs and caregivers. Mileage between visits is reimbursable and you will stock your vehicle with necessary supplies.

Typical schedule is Monday to Friday, 8 AM to 5 PM after orientation. Most CNAs see 5–7 patients per day with 45–60 minutes per patient, across home and facility settings.

Qualifications

• High school diploma or equivalent with CNA training and 12 hours of in-service training supervised by a Registered Nurse on a rolling 12-month basis.
• Current certification with State Board of Nursing, or current registration with Department of Facilities Service in NC/SC.
• No prior hospice/home health/oncology experience required; two years experience preferred.

Responsibilities

• Provide direct patient care such as bathing, peri-care, dressing, mouth care, assisting to toilet, incontinence care, repositioning in bed, and meals.
• Document care at bedside on a company-provided cellphone.
• Chart in the facility EMR when patients are in facilities.
• Care for patients wherever they consider home (home, assisted living, skilled nursing facilities) and tailor care to diverse backgrounds.
• Coordinate visit times with patients or caregivers and train caregivers to provide personal care within scope.
• Assist with tidying area, changing bed linens, and ensuring adequate supplies in the vehicle.

Hospice CNA

Schedule:

• After your orientation period - your typical schedule will be Monday to Friday, 8 AM to 5 PM.

Case Load:

• On a usual day, most of our CNAs see 5-7 patients.


• You will typically spend 45 minutes to 60 minutes with each patient.

What you’ll be doing:

• You’ll see your patients wherever they consider home (personal residence, assisted living facilities, skilled nursing facilities). We provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds.


• If you have patients in facilities, you may be responsible for charting within the facility EMR.


• You will care for patients and complete documentation at the bedside on a company provided cellphone.


• You’ll provide direct patient care such as bathing, peri-care, dressing, mouth care, assisting to toilet, incontinence care, repositioning in bed, and assisting with meals.


• You will tidy the immediate area and change bed linens for the patient.


• Expect to spend several hours a day in your car driving between patients. Your mileage between patient visits is reimbursable.


• You’ll need to make sure you maintain an adequate level of supplies in your vehicle to provide patient care. You’ll pick up new supplies at the branch office in your area.


• You’ll be responsible for working closely with each RN case manager to communicate patient status changes, as well as patient or family concerns.


• Part of your visits will consist of training the caregivers to provide personal care to their loved one, while remaining within your scope of practice.


• You’ll be responsible for coordinating visit times with the patient or caregiver for all assigned patients.

What we’re looking for:

• 1. Education: High school diploma or equivalent and completion of CNA course. Must attend 12 hours of in-service training supervised by a Registered Nurse on a rolling 12-month basis. Competency of the CNA is assessed in the home every 14 days. The CNA does not need to be present unless there is an area of concern. Otherwise, an onsite evaluation occurs once a year. Has not had a lapse of 24 consecutive months where the aide has not provided services for compensation.


• 2. Licensure / Certifications: Current certification with State Board of Nursing, or current registration with Department of Facilities Service in the State of North Carolina/ South Carolina.


• 3. Experience: No prior experience required. Two years experience with hospice, home health or oncology preferred.
