Job summary

Garden City Healthcare Center in Modesto, CA is seeking motivated and compassionate Certified Nursing Assistants (CNAs) to join our Full-Time NOC shift team. New grads are welcome and supported in a resident-centered setting.

As a CNA, you will assist residents with ADLs, monitor vital signs, document care, and participate in shift reports. We offer competitive pay, full benefits, and opportunities for growth within our SNF.

Qualifications

• Must possess a current Certified Nursing Assistant (CNA) license in the State of California.
• CPR certification preferred.
• Demonstrates compassion, dependability, and teamwork to deliver resident-centered care.

Responsibilities

• Assist residents with activities of daily living (ADLs) such as bathing, grooming, oral care, dressing and toileting.
• Measure and record vital signs and weights as instructed.
• Document care provided in flow sheets, charts, and notes.
• Report changes in residents' conditions promptly to the Charge Nurse or Supervisor.
• Participate in shift reports and hand-offs.

General Purpose

The primary purpose of the Certified Nursing Assistant (CNA) position is to provide assigned residents with routine daily nursing care and services in accordance with each resident’s assessment, care plan, and facility policies, under the supervision of licensed nursing staff.

Essential Duties And Responsibilities

• Report all changes in residents’ conditions to the Charge Nurse or Nurse Supervisor promptly


• Accurately document care provided in flow sheets, charts, and notes


• Assist residents with activities of daily living (ADLs), including bathing, grooming, oral care, dressing, and toileting


• Measure and record vital signs (TPRs) and weights as instructed


• Monitor residents routinely to ensure comfort, safety, and personal care needs are met


• Record food and fluid intake and report changes in appetite


• Participate in shift reports and communication hand-offs


• Assist with restorative and rehabilitative care as directed


• Maintain a calm, respectful, and resident-centered environment


• Cooperate with nursing staff and other departments to ensure quality care


• Perform additional duties as assigned

Qualifications

Certificates / Licenses

• Current Certified Nursing Assistant (CNA) license in the State of California


• CPR certification preferred

Work Environment & Physical Requirements

• Frequent standing, walking, lifting, and assisting residents
