Job summary

Freedom Village Bradenton is seeking a dedicated MDS Assistant with active CNA certification to join our skilled nursing team. You will provide clerical support to the MDS/Care Plan office while coordinating care conferences and ensuring accurate resident documentation.

Your role includes managing insurance updates, scheduling care plan meetings, and guiding CNAs on documentation best practices within Point-Click-Care or Matrixcare environments.

Qualifications

• Active, unencumbered CNA certification in good standing.
• Strong clerical/administrative skills: typing, filing, scheduling, data entry.
• Familiarity with MDS/RAI process, care planning, and EHR systems preferred.
• Proficiency in Microsoft Office (Word, Excel, Outlook).

Responsibilities

• Track and process managed care insurance updates, authorizations, and notifications.
• Schedule care plan meetings and coordinate calendars with residents and team.
• Maintain accurate logs of insurance authorizations and upcoming expirations.
• Serve as CNA documentation champion and provide training on documentation practices.
• Assist in onboarding CNAs and support audits related to CNA documentation.
