Job summary

Rainbow Community Care is seeking a dedicated Full Time Certified Nursing Assistant to join the Inpatient Center in Johnson Creek. The position runs 11am to 8pm, four days per week, providing hands-on support to patients and families in a hospice/palliative setting.

You will assist with mobility, daily care, meals, and documentation while maintaining compassionate communication and adherence to the plan of care. Previous hospice exposure is preferred and reliability is essential.

Qualifications

• Current Wisconsin Nursing Assistant Certification is required.
• Minimum of 6 months CNA experience preferred.
• Hospice experience preferred.

Responsibilities

• Assists with ambulation, transferring, and changing position of patients.
• Assists the patient with personal grooming, toileting, incontinence care, bathing, skin care, nail care, dressing, transferring and walking.
• Assists with preparing meals and assists patients with eating and drinking.
• Gives emotional and psychological support to the patient/family.
• Maintains a clean, safe and comfortable environment. Performs light housekeeping duties as assigned.
• Reads, understands, and follows the plan of care.
• Maintains up-to date patient records and documents cares provided to the patient.
• Reports changes in patient condition according to Rainbow policies.

Purpose of the Position

Rainbow Community Care is currently accepting applications for a dedicated Full Time CNA to work at the Inpatient Center in Johnson Creek. This person will work 11am to 8pm four days per week.

Key Responsibilities

• Assists with ambulation, transferring, and changing position of patients.

Professional Qualifications

• Valid Wisconsin driver’s license and proof of current car insurance are required upon hire and must be maintained throughout employment.
• Must be able to drive to and from other office/meeting locations as necessary.
• Current Wisconsin Nursing Assistant Certification is required.
• Minimum of 6 months CNA experience preferred.
• Hospice experience preferred.
