Hours: Full-time | 0.7 FTE (28 hours/week) | PM Shift | Weekdays | 3p-11:30p

Includes weekend (every other) and holiday (3/year) rotations

No mandating

Low ratios; 6-8:1

Sign On Bonus: $3,000

Key Responsibilities

• Assist patients with ambulation, changing position, and moving to/from sitting and standing.
• Help with bathroom functions like toileting and incontinence care.
• Help bathe, shower, dress, provide oral care, and groom.
• Assist with eating and drinking.
• Perform post mortem care.
• Obtain vital signs. Assist with treatments.
• Anticipate and respond to patient needs and round on patients.
• Provide homemaker assistance. Perform light housekeeping such as changing linens, doing laundry, tidying room, and removing trash.
• Deliver patient supplies.
• Deliver food and beverages.
• Read, understand, and follow the plan of care. Document cares and changes in patient condition in electronic health record.
• Document and report significant changes in condition or unusual incidents to appropriate interdisciplinary team members such as client abuse/neglect, suicidal intent, harmful coping mechanisms or violence, etc.
• Reinforce patient and family education.
• Participate in meetings to plan and evaluate patient/family services.
• Precept new staff and students.
• Delegate tasks to volunteers as appropriate.

You Bring (Qualifications)

• High school diploma or GED, or equivalent combination of education and work experience.
• Graduation from an accredited Certified Nursing Assistant program.
• Current, unrestricted Nursing Assistant Certification in state of employment.
• One-year personal care experience, hospice, or home health, preferred.
• Must be at least 18 years of age.
• Basic Microsoft Office skills.
• Experience using electronic health records.
• Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property.
• Ability to use proper body mechanics, safe patient handling, and standard precautions.
