Job summary

Lehigh Valley Health Network is seeking a CNA to provide hands-on personal care and ADLs per the care plan under the direction of a nurse or therapist. You will contribute as a member of the care team in a home care/hospice setting.

The role requires a High School Diploma or GED, 1 year healthcare experience, and CNA certification upon hire. You will drive to patient homes and support terminally ill patients and families. Day shift and on-site assignments available.

Qualifications

• CNA certification obtained upon hire.
• 1 year experience in a healthcare setting as a nursing assistant or nursing aide.
• Knowledge of medical terminology.
• Strong time management skills.
• Ability to drive to patients' homes and manage schedule to meet patient needs.
• Ability to work in patient homes and support terminally ill patients and families.

Responsibilities

• Provides personal care and related activities within scope of practice for designated patients in accordance with the written plan of care.
• Assumes responsibility for and completes routine and complex assignments for assigned home care/hospice patients within the allotted period, including documentation and communication with the care team.
• Observes changes in patient functional status and personal care needs; reports changes to supervising clinician or clinical manager.
