Job description

In My Home Hospice Care is seeking Certified Nursing Assistant (CNA) in the Phoenix, West Valley areas.

This role provides individualized, quality care to patients (bathing, grooming, and personal care) and develops personal connections that support both patient and family. CNAs work under the supervision of a registered nurse and are part of an interdisciplinary home care team. Care delivery primarily takes place in the patient's home or in a facility where the patient receives care.

Responsibilities
  1. Develops and maintains therapeutic relationships.
  2. Provides accurate observation of patient needs.
  3. Provides quality nursing care.
  4. Provides effective pain and symptom management.
  5. Promotes patient safety.
  6. Provides effective patient, family, and caregiver teaching.
  7. Supports the death and dying process.
  8. Collaborates in providing patient care.
  9. Creates timely and accurate documentation.
  10. Utilizes resources effectively and efficiently.
  11. Maintains and enhances professional skills.
  12. Adheres to high standards of personal and professional conduct.
