Job summary

New Age Hospice is seeking a Certified Nursing Assistant (CNA) or Hospice Aide to provide in-home patient care across League City, Dickinson, Texas City, La Porte, and South Houston. You will work under an RN, assist with transfers, and monitor patient needs in the home setting.

The role requires reliable transportation and completion of an approved CNA/HHA program, with preference for experience in hospice or long-term care. Travel to local communities is expected.

Qualifications

• Education: High School Diploma Preferred; has completed one of the listed Medicare/nurse aide programs.
• Experience: At least one year preferred as Hospice Aide or Nursing Assistant in healthcare settings or completion of CNA/HHA program.
• Skills: Ability to read, write and verbally report clinical information to patients, family and hospice staff.
• Transportation: Reliable transportation, current auto insurance and driver’s license.

Responsibilities

• Provide direct patient care under RN direction and according to the Aide Plan of Care.
• Assist patient transfers and ambulation per Hospice policy.
• Document and report changes and care rendered to ensure continuity of care.
• Maintain infection control and a clean, safe home environment.
• Communicate respectfully with patients, families and staff.
• Support nutrition and hydration for patients as directed.

Job Title: Certified Nursing Assistant (CNA) or Hospice Aide

Coverage Area: League City, Dickinson, Texas City, La Porte, and South Houston area.

Job Summary: Primary function is to provide personal healthcare and related services to the patient in their place of residence; to assist in providing a safe and clean environment; work cooperatively with the patient, and family; and share observations and problems with the supervisor. Works under the direction and supervision of a registered nurse. Looking for an Aide who will cover North Houston and surround areas.

Job Qualifications:

• Education: High School Diploma Preferred. Has successfully completed one of the following: A) Training program/competency evaluation as specified in §418.76(b)(c) of the Medicare regulations; B) Competency evaluation program that meets §418.76(c); C) Nurse aide training/competency evaluation program approved by the state and listed in good standing on the state nurse aide registry; D) State licensure program that meets requirements of §418.76(b)(c).
• Experience: At least one year experience preferred as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long‑term care facility or completion of Certified Nursing Assistant or Certified Home Health Aide Program.
• Skills: Must be able to read, write, and verbally report clinical information to patients, representatives, and care givers, as well as to other hospice staff. Demonstrates interest in the welfare of the ill and elderly. Successful completion of the Aide Competency Evaluation Skills Checklist that meets §418.76(c) and written Skills Test.
• Transportation: Reliable transportation and current auto insurance. Current driver’s license.

Environmental and Working Conditions:

Works in patients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather.

Physical and Mental Effort:

Prolonged standing and walking required. Ability to lift up to 50 pounds and move patients. Requires working under some stressful conditions to meet deadlines, to identify patient needs, to make quick decisions, and to meet patient and family needs. Requires eye-hand coordination and manual dexterity. Ability to use durable medical equipment in the home.

Essential Functions:

1. Promotes the philosophy, mission and administrative policies of the Hospice by presenting a positive image.
2. Provides direct patient care to patient under direction of the RN and according to the Aide Plan of Care.
3. Provides necessary skills to assist the patient with safe transfers and ambulation per the Hospice policy.
4. Provides necessary skill to appropriately report changes and document pertinent information and care rendered to patient to ensure continuity of care.
5. Practices accepted infection control principles. Provide a clean, safe, and comfortable environment.
6. Promotes positive, supportive, respectful communication to patient/family and other employees.
7. Provides an environment which promotes respect for patient, privacy, and property.
8. Provides skills necessary to assist the patient with proper nutrition and adequate fluid intake.
9. Contributes to the management and efficient operation of the Hospice and demonstrates effective time management skills.
10. Participates in the QAPI program.
11. Provides skills necessary to perform treatments and procedures according to the Hospice policy.
12. Promotes the Hospice philosophy and administrative policies to ensure quality of care.
13. Performs other duties as assigned.
