Position: CNA/ Hospice Health Aide - PRN

Service Areas: Clear Lake & La Porte, Baytown

Responsibilities:

1. Provide exceptional care in coordination with the nursing care team.
2. Maintain accurate patient records, including changes in doctor’s orders, diet, or personal activities.
3. Assist with patient meals, feeding, and activities of daily living.
4. Report weekly to the case manager and communicate any significant changes in patient conditions.
5. Educate patients and their families.
6. Prepare timely and accurate clinical notes.
7. Communicate effectively with healthcare providers and team members.

Qualifications:

1. Valid CNA license in Texas.
2. At least one year of hospice experience.
3. Ability to lift up to 50 pounds.
4. Availability to work weekends, holidays, and evenings occasionally.
5. Strong communication and interpersonal skills.
6. Experience with HCHB EMR is preferred but not required.
