Job summary

Hearth Hospice is hiring a Per Diem Hospice Aide (CNA) to join our compassionate team, delivering hands-on care with dignity at the end of life. You’ll work closely with our nursing staff to meet physical, emotional, and personal needs in the patient’s home.

The position requires a High School Diploma or GED, current CNA certification, and at least 1 year of CNA experience in acute or long-term care. CPR certification and reliable transportation are also needed.

Qualifications

• High School Diploma or GED required.
• Current unencumbered CNA certification in the state.
• Minimum 1 year CNA experience in acute or long-term care; hospice experience a plus.
• CPR certification if state mandated.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide personalized care by following the plan of care and attending to patient and family requests with compassion.
• Assist with personal care, including bathing, hygiene, changing bedding, clothing, and grooming while preserving dignity.
• Demonstrate exceptional customer service and support to patients and families.
• Support families with guidance and knowledge to care for loved ones at end of life.
• Maintain accurate documentation in the patient home and agency records.

Responsibilities

• Provide Personalized Care: Follow the carefully crafted plan of care to ensure the comfort and well‑being of our patients. Attend promptly to the requests of patients and their family/caregiver, offering compassionate and unwavering support.
• Assist with Personal Care: Support patients with various personal care needs, including bathing, oral hygiene, changing bedding, clothing, and shampooing. Your presence during these intimate moments contributes significantly to their overall comfort and dignity.
• Be Dedicated to Customer Service: Emphasize exceptional customer service, going above and beyond to ensure optimal patient satisfaction. Your dedication to providing outstanding care will be valued and celebrated as part of our team.

Qualifications

• High School Diploma or G.E.D. required.
• Holds current unencumbered certificate in the state working as a C.N.A.
• Minimum one (1) year experience as a C.N.A. in an acute care or long term care setting. Hospice exp a plus!
• Possess and maintains current CPR certification if state‑mandated
• Current driver's license, vehicle insurance, and access to dependable transportation.

Salary

$16.00 - $18.50 per hour (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
