Job summary

Tapestry Hospice & Palliative Care in North Georgia is seeking a compassionate Hospice Aide/CNA to join our family-owned team. You will be part of a dedicated care family, providing comfort and dignity to patients and their loved ones while collaborating with doctors, nurses, social workers, and other caregivers.

Your role involves following a carefully planned Care Plan, reporting to the RN Case Manager, and communicating needs to the Interdisciplinary Team, ensuring compassionate care and

Qualifications

• Georgia CNA license current.
• CPR and First Aid certification current.
• 1–2 years hospice or related care experience.
• Utmost compassion and integrity.
• Driver's license, insurance, and vehicle.
• Excellent time management skills.

Responsibilities

• Follow a personalized care plan for each patient.

Description:

As a Hospice Aide/Certified Nursing Assistant (CNA), you are one of the most vital components of the Interdisciplinary Team! You are the heart, the eyes, and the key communicator of your patient’s needs. Your role is to follow a carefully planned out, personalized Care Plan for each patient you are assigned and to report and work closely with the RN Case Manager to ensure the care of the patient is completed and expectations exceeded. You will effectively interact with all members of the Interdisciplinary Team as well as with your patients and their loved ones while maintaining the highest standards of care and professional nursing clinical competency.

Minimum Requirements:

• Current CNA License in the State of Georgia
• Current CPR & First Aid Certification
• 1-2 years hospice experience (other candidates may be considered with experience in assisted living, home health, long term care)
• Utmost compassion & integrity
• Driver’s license, insurance and vehicle
• Excellent Time Management Skills
