Job summary

Santè is seeking a Certified Nursing Assistant to provide personal care to patients in residence under RN supervision, in Mesa, Arizona. You will adhere to evidenced-based practices, support holistic care, and help maintain continuity of care in a nurturing environment.

Responsibilities include assisting with admissions, transfers, and discharges and reviewing care plans daily. Requires CNA license, CPR, and fingerprint clearance, with 6 months of CNA/LNA experience preferred.

Qualifications

• Active CNA license/certification.
• Current CPR and fingerprint clearance.
• Minimum six months CNA/LNA experience preferred.
• Recent Long-Term Care/SNF or Transitional Rehab experience.
• LTC/SNF experience preferred.

Responsibilities

• Assist guests/patients during admissions, transfers and discharges per care plan.
• Review care plans daily to determine changes in daily care routine.
