Must be a Certified Nurse Assistant in the state of Connecticut

Position Purpose:

Our home health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements, and our policies and procedures.

What You Will Do:

  • Follow care plans developed by RN.
  • Report client changes in condition to RN, as described on care plan.
  • Assist clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
  • Assist clients with household tasks including planning and preparing meals, feeding, light housekeeping, changing linens, making beds, and laundering clothes and bedding.
  • Measure and record client’s oral, rectal, and axillary temperatures, pulse, respiration, and blood pressure as ordered.
  • Perform range of motion and other simple procedures as an extension of therapy services as assigned.
  • Assist clients in self-administration of medication, when assigned by an RN and documented in the clinical record. Note: Home Health/Hospice Aide shall not administer medication of any kind.

Minimum Qualifications:

  • High School diploma or GED required.
  • Active CT CNA certification in good standing.
  • Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
  • Documentation of at least one year’s employment as an aide in an institution, home care, or hospice setting.
  • Must have a current Driver’s license and reliable transportation to and from the work site.
