As part of our interdisciplinary Home Care team, our CNAs (Home Health Aides) provide patients with care in the comfort of their own home and/or similar setting. We serve patients throughout Milwaukee County.

Home Health Aide Responsibilities:
  1. Support patient care by reviewing care plans prior to providing care, and notify the Case Manager of any issues or concerns.
  2. Assist with daily living activities such as bathing, dressing, transfers, range of motion exercises, and toileting under the supervision of the Case Manager.
  3. Explain the care being provided to patients and involve them in their cares.
  4. Complete mobile care tasks upon arrival and departure from patients' homes, in accordance with agency policies, and maintain accurate mileage logs.
  5. Work cooperatively with patients, families, care providers, and agency staff, responding to patient needs and reporting pertinent information timely.
  6. Perform home maintenance tasks as per the care plan, including house cleaning, laundry, meal preparation, and transportation.

Minimum Requirements:
  • Wisconsin CNA certification valid for Federally Certified Home Health Agencies and Hospices
  • Prior experience working with the public
  • Current/valid Wisconsin Driver's License, reliable and insured vehicle

Position details: Part-Time, 28 hours/week, 8am-4:30pm, includes weekend and holiday rotations. Hourly, non-exempt.
