Summary:
- The Penn Medicine at Home (PMAH) Home Health Aide is responsible for implementing a Plan of Care, assisting patients and families with personal care tasks, and activities of daily living. The individual performs home health or hospice visits throughout a designated service area. Timely and accurate documentation of patient care is essential.
Responsibilities:
- Patient Care – Perform or assist with bathing, oral hygiene, skin and hair care, prepare light meals, assist with eating, dressing, and use of toileting devices. Empty Foley catheter drainage bags and record amounts when applicable.
- Exercise and Transfers – Assist with home exercise programs, ambulatory needs, and bed-to-chair transfers. Turn patients in bed for comfort.
- Patient Environment – Perform household tasks as specified, assist with medication management, and provide respite care as needed.
- Documentation – Document care accurately and timely, complete all required reports, and verify submissions. Support government reimbursement documentation.
- Communication – Report any unusual findings or patient status changes to the primary nurse or supervisor. Facilitate communication among family, RN Case Manager, and scheduling staff.
- Plan of Care – Follow the written care plan, providing care only as outlined unless instructed otherwise. Review biweekly plans before providing care.
- Employee Travel – Provide own transportation and meet state vehicle operation requirements.
- Education – Attend annual in-services, complete at least 12 hours of continuing education annually, and meet all competency requirements.

Education or Equivalent Experience:
- H.S. Diploma/GED (Required)
- 1+ years recent experience as a Nursing Assistant or Home Health Aide, or 0-1 years with completed training and clinical coursework with a GPA of 3.0 or higher.
