Your responsibilities include assisting with daily living activities, monitoring health, and reporting changes to the care team.

Responsibilities

• Provide skilled, compassionate care to patients in their homes.
• Assist with daily living activities such as bathing, dressing, and grooming.
• Monitor and report changes in patients' condition to the healthcare team.
• Develop and implement personalized care plans that align with patients' goals.
• Collaborate with other healthcare professionals to ensure high-quality care.
• Provide supportive care to patients, including ADLs.
• Administer basic care, such as bathing, dressing, and toileting.
• Monitor patients' vital signs and report changes to the healthcare team.
• Assist with medication management and administration.
• Maintain accurate and detailed records of patient care and services provided.
