A Place at Home - Kansas City South is seeking a dedicated Home Care Coordinator/CNA to join our team. You will schedule and coordinate caregiver assignments, provide direct care to clients, and build strong relationships with caregivers and families.

This full-time role offers a 40-hour workweek, primarily Monday–Friday, with competitive hourly pay and benefits such as paid holidays and paid vacation. Excellent growth opportunities exist in a locally owned home care agency.

Qualifications

• Must hold a valid CNA certification in KS or MO and have hands-on caregiver experience.
• Experience coordinating caregiver schedules and client care, preferably in home care.
• Strong written and verbal communication; organized with attention to detail.

Responsibilities

• Manage and maintain caregiver schedules to ensure reliable client coverage.
• Match caregivers with clients based on skills, availability, and needs.
• Identify and resolve scheduling gaps, call-offs, and conflicts.
• Provide direct, non-medical personal care when needed.
• Assist field staff with client routines, mobility, and tasks per policy.
• Conduct caregiver introductions and initial visits to foster good relationships.
• Build and maintain positive relationships with caregivers, clients, and families.
