Job summary

Qualifications

• Effective communication skills including reading, writing and following directions.
• Ability to perform physical tasks such as lifting, bending and standing.
• Maintain certification with DFS.

Responsibilities

• Provide direct patient care as assigned in home care settings.
• Assist with personal hygiene and daily living activities.
• Support patients with transfers, repositioning, and ambulation.
• Provide basic nursing care and execute plan of care per supervising clinicians.
• Maintain accurate records and report changes in patient condition.
• Complete task entries in EVV time sheets per agency policy.
