Extended Family Home Care Services, LLC in Southwest Florida is actively hiring PRN CNAs and Home Health Aides to deliver in-home patient services. The role offers flexible scheduling with weekday, weekend, and holiday shifts and competitive pay with benefits.

Qualifications

• High school diploma or equivalent is required.
• CNA/CHHA certification is required.
• AHCA Level II clearance is required.
• CPR/First Aid certification is required.
• TB test within 12 months or chest X-ray within 5 years.
• Drivers license with insured vehicle is required.

Responsibilities

• Provide personal care and related home health services under RN supervision.
• Follow the plan of care to deliver safe, competent care.
• Assist with activities of daily living and mobility as approved.
• Document visits and prepare weekly visit reports.
• Coordinate with care team and other agencies as needed.
• Report changes in client condition promptly to supervisor.
• Maintain a safe, clean, and healthful home environment.
