Lifeworks is hiring a full-time Certified Nursing Assistant to support adults with ID/DD in a residential setting in Westwood, MA. The role emphasizes hands-on personal care, emotional support, and daily living assistance in a trauma-informed, dignity-centered environment.

Candidates should have prior experience with ID/DD, be able to pass background checks, hold CPR/First Aid and MAP certification, and have a valid MA driver’s license. Strong communication is essential.

Qualifications

• Associate degree (A.A.) or equivalent from a two-year college or technical school.
• CPR and First Aid certification.
• MAP certified.
• Fingerprint background check, CORI, DPPC and driving background checks.
• Experience working with individuals with ID/DD.
• Valid Driver's License in the state of residence.
• Excellent verbal and written communication skills.

Responsibilities

• Provides preventative health maintenance (BP, weight, health education) and handles daily health issues.
• Provides educational training, counseling and physical assistance to achieve ISP goals.
• Acts as resource person on health issues for staff, individuals and families.
• Maintains medical and health progress records and tracks changes.
• Audits medications agency-wide and ensures proper administration.
• Administers prescribed meds per nursing techniques and MAP regulations.
• Observes patients, records significant conditions, notifies physicians as needed.
• Attends meetings, in-services and medical appointments as required.

Lifeworks is hiring a full time CNA to work with Individuals with ID/DD in one of our Residential Programs. We are a nonprofit human service agency that provides adults with developmental disabilities the opportunity to live engaged and meaningful lives. We're searching for an enthusiastic individual who will bring their personality, an independent work ethic, and a strong belief in enhancing the lives of those we serve. Candidates must have related or direct experience working with the community we serve.

This role supports men in a residential setting and involves providing hands‑on personal care, emotional support, and daily living assistance in a trauma-informed, dignity-centered environment. This position is assigned to a single-gender residential program and requires compliance with same‑gender staffing guidelines.

Essential Duties

• Provides preventative health maintenance (blood pressure check, monitoring weight, health and education) and treat any daily health care problems (including first aid and emergency problems).
• Provide educational training, counseling and physical assistance when necessary to achieve ISP goals, and provides regular in-service training.
• Act as an agency wide resource person to program staff, individuals, families, and care providers regarding health issues and concerns.
• Maintain individual records related to medical and health progress, difficulties, and changes.
• Audits medications agency wide.
• Administers prescribed medications and treatments in accordance with approved nursing techniques and MAP regulations.
• Observes patient, records significant conditions and reactions, and notifies Director or Physician of patient’s condition and reaction to drugs, treatments, and significant incidents.
• Takes temperature, pulse, blood pressure and other vital signs to detect deviations from normal and assess condition of patient.
• Attends meetings, in-services, significant medical appointments, and case conferences as necessary.
• Other duties and/or assignments to work/program locations based on the needs of the company.

Requirements

• Associate degree (A.A.) or equivalent from two-year college or technical school.
